A federal survey found that more than 45 million U.S. adults had some form of mental illness in 2011.

What is it like to live with mental illness? Whether you have been diagnosed or a loved one has, KPCC wants to know what it's like, and how mental illness affects your life.

Help us learn more about how we should report on the story, and shape a conversation to come by telling us about your personal story with mental illness. Just fill out the short form below, or tweet at us with the hashtag #LivingWith.
